Registrar's Office Forms Index - Just Type and Print

Unless otherwise noted, you must type in and print out the form you need, secure all required signatures, and then submit it to the Registrar's Office.

Address Change Form
Print this form out and submit it to the Registrar's Office to notify of a change in mailing address and/or local address. 
Cancellation of Registration Form
Print this form out and submit it to the Registrar's Office to cancel your registration (and financial aid, if applicable) prior to the first day of class.  
Enrollment Verification Request Form
Print this form out and submit it to the Registrar's Office to request verification of enrollment or graduation. 
Departmental Transfer Credits Substitution Form
Complete this form within the second semester after transferring to AAMU to receive credit for courses taken at the college of transfer.

Major Declaration or Change Form
This form is to declare your major/minor or notify of a change in major/minor. Print this form out, complete it, and leave it with the Dean of the school of your new major/minor. They will forward it to the Registrar's Office. 
Name Change Form
Print this form out and submit it to the Registrar's Office to notify of a change of name or incorrect SSN. 
Record Correction Form
Print this form out and submit it to the Registrar's Office after repeating a failed course to have your academic record corrected, or for correcting other problems with your record.  
Transcript Request Form
Print this form out and submit it to the Registrar's Office to request an official transcript 
Transient Student Form
Print this form out and submit it to the Registrar's Office to receive credit for courses taken at another institution as a transient student.  
Visiting Student Form
Print this form out and submit it to the Registrar's Office to receive credit for courses taken as a visiting student.  
Withdrawal Clearance Form
Print this form out and submit it to the Registrar's Office to withdraw from the university.
